Departmental Volunteer Service Hour Policy

To begin the Volunteering Process, please emailÌý leadandserve@fau.edu Ìýto obtain the Human Resource form. All interested volunteers must have a completed background check to volunteer on the ´óÏó´«Ã½ campuses.

Volunteer Risk Waiver Ìý(For ´óÏó´«Ã½ and Non-´óÏó´«Ã½ Volunteers)

Volunteer Registration Ìý(For ´óÏó´«Ã½ and Non-´óÏó´«Ã½ Category One Volunteers)
This form is to be completed by all Category One Volunteers.

Parent/Guardian Authorization for the Treatment of MinorsÌý
This form is to be completed by a parent or guardian of all Category One and Category Two volunteers who are minors.

Academic Service-Learning Risk WaiverÌý
This form is a risk waiver that must be signed by all ´óÏó´«Ã½ students completing an Academic Service-Learning project as part of their course.
