Report of Outside Employment/Professional Activity

As we begin the new fiscal year, HR would like to remind all full time employees that University policy requires that anyone engaged in outside employment/professional activity during the new fiscal year (July 1, 2017 – June 30, 2018) must report these involvements. This is to ensure that participation in these activities doesnot create a conflict of interest with one’s primary duties here at ý.

Before assuming or continuing any other interests outside of ý please complete the “Report of Outside Employment or Professional Activity” form(s); located at:
